Job Description:

  • $10,000 Sign On Bonus with 2 year commitment (4 Installments)
    $3,000 Relocation bonus for qualified candidates.

  • Practicing across the spectrum of care, including ambulatory outpatient clinic and inpatient, and serve as an integral part of the healthcare team involved in all aspects of transitions of care.

  • Delivers medication therapy management services and proactively intervenes in areas of medication use to optimize patient outcomes.

  • Participates as a member of the Transitions of Care clinic to provide integrated clinical and distributive pharmacy services (e.g. warfarin anticoagulation monitoring, diabetes management, heart failure management, COPD management, etc.), interprofessional pharmacy practice, direct patient care programs, expert experiential pharmacy student and pharmacy resident training, and leadership of quality improvement and practice advancement initiatives.

  • Performs complete medication order scrutiny over all aspects of the medication reconciliation care continuum, including comprehensive chart review as well as monitoring of patients to identify, prevent, or mitigate medication-related problems.

  • Actively functions a member of clinical pharmacy services and hospital-wide clinical committees to improve on providing best quality care for cardiac patients.

  • Actively participate as a member in the Residency Advisory Committee, Transitions of Care rotation preceptor, and residency preceptor for patient care and medication use evaluation projects.

  • Completes critical medication order scrutiny, drug profile review, and patient monitoring to identify, prevent, or mitigate medication-related problems.

  • Verifies and enters medication orders, proactively communicating with medical staff to resolve problems.

  • Provide direct patient care, including pharmacotherapy assessment, therapeutic monitoring, pharmacokinetic dosing, and transitions-of-care.

  • Provides accurate, safe, and timely medication therapy, and promotes cost-effective drug therapy.

  • Precept, mentor, and educate pharmacy students, residents, and staff, and provide continuing education to interdisciplinary team members.

  • Working knowledge of the American Society of Health-System Pharmacists Accreditation Standards for Pharmacy Residency Programs Required

  • Ability to coordinate and lead small groups of people in determining policy, implementing procedures and demonstrating progress Required

  • Leadership qualities essential to operate effectively within the institution and advance the practice of pharmacy Required

  • Must be able to read, write, speak, and have good comprehension of conversational English Required

  • Basic computer skills and knowledge of Microsoft Office Required

  • Ability to define operational or program problems/needs and solutions Preferred

  • Ability to integrate clinical and distributive pharmacy services with quality improvement, teaching, research, and professional development activities Preferred

  • Expertise using Cerner pharmacy solutions and services Preferred

  • Expertise using Pyxis automated dispensing technology Preferred

  • Working analytical and data management skills Preferred

  • Working knowledge of Microsoft Office Outlook, Word, Excel, and PowerPoint Preferred

  • Working knowledge of the accreditation requirements of Det Norske Veritas and International Organization for Standardization quality management systems Preferred

  • Working knowledge of the American Society of Health-System Pharmacists National Practice Standards Preferred

Education:

  • Bachelor's in Pharmacy Required

Work Experience:

  • Completion of Post Graduate Year One (PGY1) residency Required AND

  • Post Graduate Year Two (PGY2) residency in specialty practice area, OR5+ years of contemporary hospital or specialty practice experience consistent with institutional needs Required

Licenses and Certifications:

  • Licensed Pharmacist (RPH) Required

$50.49 - $93.91
