· Contract Length: Five (5) year Initial Term with a one-year autorenewal thereafter
· Base Salary: $275,000 guaranteed for the first two contract years, then predicated on the Base RVU Target
· Base RVU Target: 5,500 per Contract Year
· Productivity Incentive: $40/RVU greater than the Base wRVU target of 5,500
· Sign-On Bonus: $75,000 forgiven over the five-year Initial Term
· Relocation: up to $10,000 or reasonable costs (not a cash benefit)
· Extender Bonus: $1 per Extender RVU
· Residency Incentive: $20,000 total payable in equal monthly installments, less taxes, forgiven over the initial term. Payment commences the first month Physician is successfully onboarded and continues through the month prior to commencing onsite work.
· Educational Loan Incentive: $25,000 per year for each year of the Initial Term paid at the end of each Contract Year
· PTO: 20 days (160 hours) + 7 UPMC Holidays per calendar year.
· CME: 5 days (40 hours) and $3,500 reimbursement per year

· Malpractice Insurance: Occurrence-based coverage
