Client Assessor

Purpose:

Is responsible for assessing new clients and reassessing existing clients. May be asked to participate in the packaging of congregate and home delivered meals as necessary.

Responsibilities:

• Use an Area Agency on Aging prescribed form to assess potential clients as assigned by the Prime-Time Office Staff
• Use an Area Agency on Aging prescribed form to reassess clients as assigned by the Prime-Time Office Staff
• Notify the Director or Prime Time Office Staff of any client found to be at high risk or of a change in any client's status
• Assist Prime Time Office Staff in the preparation of client files
• Package food for distribution by home delivered mean staff
• Other duties as assigned
• High school diploma or equivalent.

Licensure, Certifications, and Clearances:

• Driver's License
