Work location: Based out of Texas Health Dallas and Texas Health Fort Worth
Work hours: Full-time, Monday – Friday, 9:00am – 5:00pm, with up to two Saturdays per month required
This is a grant funded position and subject to the award and continuation of the contract Here’s What You Need:
· High School Diploma or Equivalent required.
· MA - Medical Assistant Certified Medical Assistant (CMA) from a nationally recognized certifying organization. Required upon hire. Other National Board-Certified Health & Wellness Coach (NBC-HWC) 18 Months required.
· 4 Years Experience in a healthcare or community health environment providing patient education, facilitating health discussions, or presenting health-related information to individuals or groups, required.
· CPI - Crisis Prevention Intervention Training 90 Days required.
· Driver’s License (required)
· BCLS - Basic Cardiac Life Support 90 Days required.
Here’s What You Will Do:
· Deliver structured group education sessions utilizing the approved THR Curriculum focused on chronic disease prevention, postpartum recovery, and healthy lifestyle practices.
· Guide participants through curriculum content related to nutrition, physical activity, stress management, chronic disease prevention, and long-term wellness.
· Facilitate discussions that encourage peer support, shared learning, and participant engagement.
· Maintain fidelity to the Centering curriculum while adapting delivery to meet the needs of diverse community populations.
· Support postpartum mothers in adopting healthy lifestyle behaviors that reduce long-term risk for chronic conditions such as diabetes, hypertension, and cardiovascular disease.
· Support outreach efforts to recruit eligible postpartum mothers and community members into lifestyle education programs.
