Job Summary:

Under the direction of the Clinical Pharmacy Manager and ED Pharmacists oversight, the Pharmacy Medication Reconciliation Technician is responsible for obtaining a comprehensive and accurate patient home medication list as part of the admissions process. This role will have a significant impact on improving safety and quality of our patients by providing physicians and other clinicians with a timely comprehensive home medication list Coordinates/assists with deliveries of code trays to the Emergency Department Actively participates in ongoing quality improvement programs focused on medication reconciliation.
Essential Functions:
-Communicates completion of medication list review and medication reconciliation to Physician, Nursing and Pharmacy staffs.
-Assists Pharmacists in preparing and distributing medications, including sterile IV products, maintaining the medication inventory, and maintaining records.
-Executes cordial and confident customer service skills via telephone or in person with all customers (Patients, Physicians, Nursing, other disciplines) encountered
-Adheres to HIPPA regulations regarding confidentiality and patient privacy
-Perform medication history assessments (medication list review and medication reconciliation) for new admissions to the hospital, confirming complete information regarding each medication identified (i.e., drug, dose, route, frequency, indication for PRN, and last dose taken- date/time).
-Verify patient allergy information and ensure complete documentation (i.e., substance, allergy category, reaction description)
-Assist and coordinate medication history assessment with Nursing team, upon request
-Document the medication history and allergy information in the electronic medical record accurately and in a timely manner.
-Participate in performance improvement activities
-Precept Pharmacy Technicians and Pharmacy Students regarding home medication interviews and documentation
-Responsible for knowledge and application of hospital-wide medication policies and procedures and state and national regulatory requirements based on clinical, operational, and regulatory standpoint to ensure areas of ownership and accountability
-Examine a patient’s medication list in the context of the current episode of care to support appropriate treatment decisions. This entails identifying which medications are relevant to the current episode of care and determining if the patient is taking them, including clarifying discrepancies regarding these medications and communicating non-adherence matters
-Perform a comparison of the updated medication list reviewing/continuing existing orders, resolving discrepancies in drug type, route, dosage, amount, frequency, duration along with the corrections of omissions and/or duplications, as well as the resolution of allergy, drug interactions and/or intolerance information during admission, transfer between care units, and discharge and sharing with a licensed provider.
-Must work ED Pharmacist and Clinical Pharmacy Manager and will report and resolve any compliance concerns in a timely manner which will be raised with respective groups Quality Management and Accreditation and Compliance, as applicable
-Collect data for number of medication admission histories completed and number of discrepancies identified and report out on a periodic basis
-Serve as a liaison for the Nursing staff for ADM issues and other medication-related issues.
-Establish a communication process with admitting Physician regarding identified issues, such as medication non-compliance.

Qualifications:

  • High School diploma required Must have one year of Pharmacy Technician experience
  • Familiarity with medical/drug terminology and abbreviations.
  • Demonstrated ability to walk and stand 70% of the time and to lift up to 35 pounds
  • Must be exCPT or PTCB certified, or a registered pharmacy intern with the state of Massachusetts
  • Must be Registered with the Massachusetts Board of Pharmacy as a Pharmacy Technician or Intern Excellent communication and computer skills
  • Physical Environment: Satisfactory physical environment.
