Home Base, a Red Sox Foundation and Massachusetts General Hospital program, is dedicated to healing the invisible wounds including post-traumatic stress, traumatic brain injury, anxiety, depression, co-occurring substance use disorder, family relationship challenges and other issues associated with Military service – for Veterans of all eras, Service Members, Military Families and Families of the Fallen through world-class clinical care, wellness, education, and research.

Home Base is currently seeking a full-time Registered Nurse to provide compassionate, patient-centered care to individuals dealing with the invisible wounds related to Military Service, including post-traumatic stress, traumatic brain injury, anxiety, depression, co-occurring substance use disorder, and family relationship challenges.

-Works collaboratively with our interdisciplinary clinical team to provide care to patients across all clinical services at Home Base, including the Intensive Clinical Program and Brain Health program.
-Managing the day-to-day medical needs of patients in the intensive programs, such as taking vitals, ordering and obtaining lab work, administering toxicology screens, providing medical condition management and triage, overseeing medication management and patient education, monitoring patients during fitness sessions, and collaborating with the treatment team in shared medical case management.
-Will conduct screening calls with patients prior to their arrival and assist with reviewing medical record across programs.

Accountable for interpreting the plan of medical care, assessment of patients' clinical decision-making regarding nursing care, assuring nursing care is provided in a safe and competent manner, providing individualized nursing care, and evaluating nursing care for groups of patients.

Essential Functions
-Maintain accurate, detailed reports, and records.
-Administer medications to patients and monitor patients for reactions or side effects.
-Record patients' medical information and vital signs.
-Monitor, record, and report symptoms or changes in patients' conditions.
-Consult and coordinate with healthcare team members to assess, plan, implement, or evaluate patient care plans.
-Modify patient treatment plans as indicated by patients' responses and conditions.

Qualifications:

Education:

Associate's Degree Nursing required or Bachelor's Degree Nursing preferred

Licenses and Credentials:

  • Registered Nurse RN - State License

Experience:

Clinical nursing experience 0-1 year required

  • Knowledge, Skills and Abilities

  • Phlebotomy

  • Familiarity with the principles and skills needed for practical nursing to provide patient care and treatment.

  • Knowledgeable of the care required by respective age groups for which care is being provided.

  • Ability to maintain confidentiality and secure sensitive information.

  • Knowledge of medical terminology.

  • Excellent verbal and communication skills.

  • Ability to accurately screen and triage acute patients.
