The Pharmacy Clinical Coordinator – Critical Care role supports the inpatient pharmacy team at Lutheran Hospital, consisting of 17 pharmacists and approximately 12 pharmacy technicians, and is embedded within a high-acuity ICU environment with 9 beds and an additional crash bed. This position blends advanced critical care clinical practice with program leadership, offering strong visibility and direct impact on clinical strategy and patient outcomes. The coordinator participates in daily ICU rounds as the clinical pharmacy expert, providing comprehensive medication management for critically ill patients, guiding complex pharmacotherapy decisions, and collaborating closely with physicians, nursing, and interdisciplinary teams. In addition to direct patient care, the role oversees and coordinates clinical pharmacy services, partners on strategic initiatives, identifies and implements service improvements, ensures compliance with practice agreements and protocols, and contributes to committees and governance efforts. A typical day includes ICU rounding and case review, supporting pharmacists in complex scenarios, leading practice improvement initiatives, collaborating with hospital leadership, and participating in meetings and quality improvement activities.
A caregiver in this role will work a primary schedule of Monday through Friday from 7: 00am to 3:30pm, with up to two work-from-home days per week based on business needs. The position includes a weekend rotation every 8th weekend (for board-certified pharmacists), alternating between day and evening shifts, as well as a 1-week on-call rotation every 4 weeks from Monday 7:00am through Sunday evening. There is no standard holiday requirement, though occasional coverage may be needed based on operational needs.
A caregiver who excels in this role will:
- Identify needs and implement standardized clinical pharmacy services across pharmacy service lines (e.g., Critical Care, Oncology).
- Provide mentoring to pharmacists, residents and students.
- Provide ideas regarding new clinical services, cost savings initiatives, and practice innovation through active participation in strategic planning efforts.
- Establish processes to evaluate quality assurance within service line.
- Identify pharmacy representation for academic responsibilities and interdisciplinary committees.
- Under the direction of the manager, direct, coordinate and provide feedback to staff regarding clinical services for assigned service line.
- Participate in interviewing and recommends selection of candidates.
Minimum qualifications for the ideal future caregiver include:
- Doctor of Pharmacy degree from an American Association of Colleges of Pharmacy (AACP) accredited university
- Current state licensure as a Registered Pharmacist (RPh) or eligible for reciprocation in current state within 90 days of hire.
- For individuals required to administer immunizations, current Basic Life Support (BLS) through American Heart Association (AHA) or American Red Cross and completion of a Board of Pharmacy approved course in the administration of immunizations or certification obtained within 90 days of hire are required.
- For individuals required to respond to adult or pediatric cardiopulmonary arrest events, current Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) Certification through American Heart Association (AHA), or certification obtained within one (1) year of hire, is required.
- Completion of a post-graduate year 2 residency (PGY2) plus a minimum of 1 year of experience providing pharmaceutical care in the relevant specialty area OR Completion of a post-graduate year 1 (PGY-1) residency plus a minimum of 3 years of experience providing pharmaceutical care in the relevant specialty area OR minimum of 5 years of experience providing pharmaceutical care in the relevant specialty area
Preferred qualifications for the ideal future caregiver include:
- Previous leadership experience
- Board certification through the Board of Pharmacy Specialties that aligns to practice area strongly preferred at time of hire and required within one year
- Clinical expertise in acute/critical care pharmacotherapy
Physical Requirements:
- Typical physical demands include ability to perform functions of the job while reaching and stretching, and manual dexterity sufficient to perform computer functions of job.

- Follows standard precautions using personal protective equipment as required.
