As a Physical Therapist on the Home Care team providing coverage over Tuscarawas County, you will utilize various exercises to help patients increase strength, range of motion, endurance and coordination within the comfort of their home. Often, your patients are dealing with pain, so sensitivity is important. Because this position requires close interaction, you have the opportunity to build strong relationships with both patients and their families. Ultimately, your unconditional support and reassurance can drive them to work hard and regain their health, allowing you to make a difference in their lives every day.
This is a PRN position. A caregiver in this role will work 8: 00am to 5:00pm but will be limited to work 48 hours in a 6-week period. This would be 4-6 hours per week.
A caregiver who excels in this role will:
- Perform initial and on-going assessments of patient condition.
- Establish, revise and evaluate plans of care and involve the patient/family according to their capabilities and desires.
- Perform therapy interventions.
- Supervise activities of Physical Therapy Assistants, students and other support personnel.
- Maintain complete and up-to-date documentation and files at all times regarding patient condition and effectiveness and timeliness of care and interventions.
- Participate in ongoing interdisciplinary assessment of assigned patients.
- Participate as a member of the hospice interdisciplinary team when necessary.
Minimum qualifications for the ideal future caregiver include:
- Bachelor’s Degree or certificate
- Graduate of a Physical Therapy Program accredited by the American Physical Therapy Association
- State Valid Driver’s License (VDL) with proof of $100,000/$300,000 coverage upon hire
- Current state licensure as a Physical Therapist (PT).
- Current Basic Life Support (BLS) Certification through the American Heart Association (AHA) or the American Red Cross
Preferred qualifications for the ideal future caregiver include:
- One year of clinical experience as a licensed Physical Therapist in an acute, rehab or skilled nursing setting

Physical Requirements:
- Ability to walk and climb stairs without assistive device and to maintain balance despite sudden load in order to support patient from falling.
- Ability to meet physical demands characterized at the heavy level defined by 1993 Leonard Matheson and Ministry of Labor.
- Ability to lift and reach with arms sufficiently to lift patient's arm and leg during therapeutic treatment program.
- Sensation of touch and manual dexterity to be able to palpate muscle tension or to monitor patient pulse.
- Visual acuity to observe patient's gross and fine motor movements in order to analyze these movements and to read all patient monitors.
- Ability to speak clearly enough to instruct patients and families.
- Ability to hear at a conversational level to understand patient's description of symptoms and response to treatment.

- Follows standard precautions using personal protective equipment as required.
