Job Description:

  • Observes and records activities and findings in the clinical record, reporting patient reactions and changes to the physician or primary Case Manager.
  • Performs evaluations to determine diagnosis, prognosis, and plan of care in consultation with the physician.
  • Determines specific interventions, duration, and frequency needed to achieve anticipated goals and outcomes. Instructs other health team personnel in phases of speech therapy relevant to patient care.
  • Selects, applies, or modifies skilled interventions consistent with state practice acts to achieve the best patient outcomes.
  • Assesses patient responses to interventions and perform reassessments as required. Documents treatment provided in medical records, updating care plans as indicated.
  • Participates in clinical record reviews, agency staff meetings, therapy meetings, and other activities as requested.
  • Provides supervision and guidance to aides, ensuring compliance with care plans and documentation requirements.
  • Instructs patients and caregivers on speech language improvement programs, including adaptive equipment and home programs.
  • Other duties as assigned.

Knowledge, Skills, and Abilities:

  • A working knowledge of community resources and an ability to refer patients and families appropriately. Required
  • Knowledge of Home Care Regulations and Third-Party Reimbursement as it impacts care delivery. Required
  • Entry level competency of occupational therapy practice, theory, principle, and applications as defined in competency department policies. {Required]
  • Basic computer skills and knowledge of Microsoft Office. {Required]
  • Must be able to read, write, and speak conversational English. Required

Education:

  • Master's Required

Field of Study:

  • in Speech Therapy

Work Experience:

  • 1+ clinical speech therapy experience Preferred
  • 1+ relevant clinical speech therapy experience Preferred

Licenses and Certifications:

  • Speech Language Pathologist (SLP) Required
  • Basic Life Support
  • CPR Cert (BLS) Required
  • Driver's License (DL) Required
  • Auto Insurance (AL) Required

Pay Range:

$31.55 - $58.69
