Job Summary

The role collaborates to enhance the delivery of patient care services along the continuum of care. The resource specialist II meets patient's needs efficiently and expeditiously by continuously improving the patient's experience, helping to ensure the institutional standards of high quality patient care; reducing cost and ensuring reimbursement.

Essential Functions

  • Research resources and programs available for patients and their caregivers
  • Coordinate legal, financial and insurance resources
  • Following referral from the Clinical Social Worker, work directly with families to complete applications for assistance and provide guidance to families about the community programs that require them to apply directly
  • Manage all transportation and parking issues for families to mitigate any barriers to accessing care for their child
  • Manage requests for documentation to ensure no interruption in a family's utilities
  • Participate in daily huddle, weekly Pediatric Hematology-Oncology staff meetings, monthly Psychosocial Care meetings within the practice and monthly Social Work staff meetings
  • Provide health education material, including hard and electronic storage system that is easily accessible for program staff
  • Assist Care Manager and Social Worker with follow up patient support calls

Qualifications

Education:

  • Bachelor's Degree in Business required OR Bachelor's Degree in Related Field of Study required
  • Can this role accept experience in lieu of a degree? No

Experience:

  • Five years of which two must be related to healthcare

Knowledge, Skills and Abilities:

  • Assessment and problem solving skills
  • Strong interpersonal skills
  • Strong customer service skills
  • Ability to work independently with minimal supervision
  • Goal oriented and accountable in a complex medical environment
  • Experience navigating Insurance Networks desired
  • Demonstrated organizational and time management skills
  • Strong oral and written communication skills
  • Good computer experience

Additional Requirements:

  • Bilingual in Spanish highly desired
