Job Summary:
The Specialist is a key member of Mass General Brigham’s Health Corps, a dedicated community-based workforce advancing the system’s Community Health Strategy. Embedded in MGB’s priority communities, Health Corps members help individuals and families access care, address health-related social needs, and manage conditions that contribute to health disparities.
This position focuses on outreach, education, and navigation related to two of MGB’s identified priority conditions: colorectal cancer and cardiometabolic disease. Community Health Workers provide blood pressure screenings, deliver basic health counseling, and support individuals in connecting to services that promote prevention, early detection, and long-term health.
This role requires deep familiarity with the strengths, cultures, and needs of the communities served, which include Chelsea, Revere, Dorchester, Roxbury, Mattapan, Mission Hill, Hyde Park, Jamaica Plain, Roslindale, Lynn, Salem, and Waltham.
Principle Responsibilities and Duties:
- Conduct outreach and engagement within assigned communities to promote awareness of colorectal cancer and cardiometabolic disease and encourage screening and preventive care
- Provide blood pressure screenings in community and clinical settings, following approved protocols
- Offer basic health counseling to support individuals in understanding risk factors and adopting strategies for managing their health
- Assist individuals with accessing primary care, screening appointments, follow-up care, and social services
- Maintain current knowledge of local resources and help community members navigate systems related to food, housing, transportation, and financial assistance
- Build and maintain trusting relationships using a culturally responsive and strengths-based approach
- Collaborate with clinical teams, community organizations, and other Health Corps members to ensure coordination of care and support
- Document screenings, referrals, and encounters in appropriate systems with accuracy and timeliness
- Participate in Health Corps trainings, team meetings, and one-on-one supervision to strengthen practice, develop skills, and align with program goals
- Support local community events, health fairs, and systemwide initiatives
- Serve as a representative of Mass General Brigham and the Health Corps in all interactions with the community and partners
- Perform other responsibilities as assigned in support of the Community Health Improvement team
Qualifications:
Education
- High school diploma or equivalent is required; associate or bachelor’s degree is preferred
- At least two years of experience living or working in one or more of the following communities: Chelsea, Revere, Dorchester, Roxbury, Mattapan, Mission Hill, Hyde Park, Jamaica Plain, Roslindale, Lynn, Salem, or Waltham
- Experience in community outreach, patient navigation, health education, or other public health roles is preferred
- Experience conducting health screenings or participating in health promotion activities is preferred; training will be provided
- EMT or Medical Assistant experience strongly preferred, particularly in patient-facing or clinical support roles.

- Strong relationship building skills.
- Must have a flexible mindset with the ability to quickly adjust daily plans and activities to accommodate urgent or unplanned community needs.
- Must be comfortable communicating and collaborating with a diverse set of staff, leaders, communities, and other stakeholders.
- Experience collecting and documenting data a plus.
- Excellent written and verbal communications skills.
- Proficiency in Microsoft Office required.
- Demonstrated ability to work effectively and provide advocacy for racial and ethnically diverse populations and communities.
- Strong commitment to social justice and health equity.
- Demonstrated commitment to impacting the care of at-risk patients.
