The Pediatric Care Management Intake Clinician (MSW) performs psychosocially focused intake for children referred to care management, with an emphasis on social risk, behavioral health, family systems, and community resource linkage. This role conducts comprehensive psychosocial assessments, evaluates safety and protective factors, coordinates benefits and services, and leads warm handoffs to longitudinal care teams. The MSW partners with RNs, CHWs, and other Pedi iCMP Staff to reduce barriers to care and improve outcomes.
Job Summary
Primary Responsibilities:
Psychosocial Intake Assessment:
-
Complete comprehensive intake assessments according to best practices and alongside CHWs when able: family/caregiver supports, developmental/educational needs (IEP/504), behavioral health history, trauma exposure, safety/risk, transportation, financial and legal considerations.

Screen for SDOH (food, housing, transportation, caregiver strain), safety concerns, and developmental needs.

Identify eligibility for public benefits (Medicaid waivers, SSI, SNAP/WIC, transportation, respite).
Care Planning & Service Navigation:
-
Develop an initial integrated care plan aligned with family goals and clinical priorities.
-
Coordinate with assigned Pedi iCMP Care Manager for behavioral health referrals (therapy, psychiatry), community resources, school collaboration, and home services.

Relay the need for authorizations and applications (DME letters of support with RN/MD, home nursing, waivers) with assigned Pedi iCMP Care Manager.
Safety, Advocacy & Legal/Ethical Duties

Conduct risk assessments; escalate acute concerns via appropriate pathways.

Mandated reporting to child protective services when indicated, document thoroughly.

Advocate for language access, disability accommodations, and educational rights.
Interdisciplinary Coordination
- Co-lead case huddles with RN partners; ensure warm handoffs to longitudinal CM/CHW.
Documentation, Quality & Equity
-
Document assessments, plans, and referrals per licensure and NCQA regulatory standards.

Contribute to quality improvement and equity initiatives.
Qualifications What You'll Bring
Qualifications:
-
Master’s Degree in Social Work (MSW) from an accredited program (or equivalent master’s in Counseling/Mental Health) required.
-
State licensure: LCSW, LICSW or LMHC or eligibility within required timeframe required.
-
2 years of experience with pediatric social work (hospital, outpatient, community and/or child welfare) required.
-
Ability to prioritize urgent needs (suicide risk, unstable housing, food insecurity, utility shutoffs, CPS involvement, etc.) required.
-
Experience with complex medical needs, behavioral health, trauma-informed care, and SDOH navigation strongly preferred.

Remote with the option to attend occasional team meetings at Assembly Row in Somerville, MA.
