Mass General Brigham's Home Hospital program began in 2017, making us one of the largest and most established programs in the country. We bring acute, high-quality, compassionate, hospital-level care to patients in the comfort of their home while increasing hospital capacity and reducing total costs.
About the Role
Our Advanced Practice Providers (APPs) play a vital role in this innovative care model, working at the top of their license within a highly collaborative, multidisciplinary team to provide comprehensive, patient-centered care supported by daily in-home and virtual visits, 24/7 remote monitoring, and advanced telehealth technology. APPs in Home Hospital manage a diverse range of acute and post-acute conditions while helping improve patient outcomes, enhancing care transitions, and redefining how high-quality hospital care is delivered.
We are seeking an experienced, flexible, full-time, 36 or 40-hour Advanced Practice Provider (Nurse Practitioner or Physician Assistant) to join our innovative team. This position is fundamentally a hybrid telehealth-based role with required local travel to patient homes for in-person care and to partnering hospital sites to support patient admissions and transitions into the Home Hospital program. Our APPs are scheduled for 12-hour shifts from 7:00 AM to 7:00 PM. It is a rotating schedule, inclusive of weekends and holidays.

Responsibilities
- Provides direct care, counseling, and teaching to a designated patient population in the ambulatory, inpatient, operative, and/or procedural setting
- Performs complete histories and physical examinations
- Orders, interprets, and evaluates appropriate laboratory and diagnostic tests
- Develops appropriate plans of care and follow-up based on the outcomes of diagnostic, laboratory, and physical examination findings
- Orders medications and writes prescriptions according to organizational and regulatory policies and procedures
- Consistently provides high quality and timely documentation including admission and progress notes, procedure notes, operative notes and discharge summaries
- Performs bedside procedures as are appropriate to the patient population
- Provides care to patients both in-person in their homes and through virtual visits—managing responsibilities such as placing orders, completing documentation (including progress notes, handoffs, and discharge preparation), billing, and maintaining consistent communication with patients and the interdisciplinary care team
- Works onsite at one of our partnering hospitals (including, but not limited to Newton-Wellesley Hospital, Brigham and Women's Faulkner Hospital, or Brigham and Women's Hospital), collaborating closely with Nurse Navigators and Operations Coordinators to evaluate and admit appropriate patients from the Emergency Department or facilitate transfers from inpatient teams
- Reviews referrals, obtains patient consent, places orders, completes documentation, manages billing, and ensures clear communication with patients and the care team to support safe transitions into the Home Hospital program
- May perform additional duties, such as precepting a small group of learners
Qualifications
Education
- Master's Degree Physician Assistant required
- Experience in lieu of a degree is not accepted
Licenses and Credentials
- Physician Assistant State License
- Generic
Experience
- 2 years of experience as a PA required
- 3+ years of acute hospital APP experience highly preferred
- Academic medical center experience a plus
