The Opportunity:
As an integral member of the Population Health Management Operations team, the Transitions of Care Clinical Pharmacist will support Medicare, Medicaid and Commercial risk patients as they transition from an inpatient hospital stay to their homes. In this role, the pharmacist will leverage clinical expertise to support patients through transitions of care, utilizing telehealth engagement to engage with patients remotely. The primary responsibility will be to ensure a seamless transition from hospital to home, optimize medication therapy, and enhance patient outcomes. The pharmacist will be expected to conduct post-discharge medication reconciliation, identify potential drug-related medical problems, and mitigate the risk of readmission through patient education and therapy recommendations to provider.
Job Summary:
Primary Responsibilities:
- Review discharge summaries and conduct remote consultations with patients, caregivers, providers, and/or outside pharmacies to complete post-discharge medication reconciliation
- Review and analyze medication orders and patient profiles to identify and resolve medication-related issues.
- Develop and implement individualized medication therapy plans to optimize therapeutic outcomes and improve adherence.
- Provide education on medication management, including proper use, potential side effects, and drug interactions by utilizing motivational interviewing to promote behavior change
- Assess compliance issues and develop plan to improve adherence
- Address affordability concerns, including, but not limited to cost-effective drug therapy regimen, simplification of medication regimen, and prescription assistance programs
- Document patient encounters, update medication lists, and communicate with providers via electronic medical record
- Collaborate with multidisciplinary teams to ensure a coordinated approach to patient care
- Responsible for drug management therapy of patients with specified disease states as part of collaborative practice agreement
- Participate in quality improvement initiatives and contribute to the development of best practices for transitions of care.
- Maintain own professional and specialized competency through continued education.
- Collaborate with colleagues on other job-related duties and projects as directed.
Qualifications:
Qualifications:

Must be a graduate of an accredited college or university with a Pharm D degree.

Licensed to practice pharmacy in the Commonwealth of Massachusetts required.

Board certification and/or PGY1 residency is required.

3+ years of experience in clinical pharmacy practice, with a focus on transitions of care preferred.

Experience functioning as a clinical pharmacist in ambulatory setting preferred.
Additional Knowledge, Skills and Abilities:

Strong clinical knowledge and experience in medication management.

Excellent communication skills, with the ability to engage patients effectively via telehealth.

Excellent interpersonal skills and ability to work collaboratively

Knowledge of healthcare systems and community resources

Must be self-directed and proactive

Proficient in Microsoft Office, including Word, Excel & PowerPoint

Ability to be flexible with working hours.
