As a Healthy Start Homecare Nurse, you will participate in a team-based care model led by the Care Coordinator, providing top-quality maternal and child healthcare to the community. You have the benefit of a flexible schedule, offering a unique opportunity to make a difference while maintaining a healthy work-life balance.
A caregiver in this position works days from 7: 30AM – 4:30PM.
A caregiver who excels in this role will:
-
Provide direct nursing care and perform accurate and complete starts of care, resumptions, recertifications and routine visits as requested and within specified time frames.
-
Perform medication reconciliation at each visit.
-
Assess patient for safety risks, abuse and neglect.
-
Serve as a primary coordinator of all disciplines for well-coordinated patient care.
-
Assess and document pain, manage pain and prioritize/document problems based on assessed needs and problems in collaboration with patient, family and health care team.
-
Maintain complete and up-to-date documentation and file at all times regarding patient condition and effectiveness and timeliness of care and interventions.
-
Escalate change in patient condition to physician as appropriate.
Minimum qualifications for the ideal future caregiver include:
-
Graduate of an accredited school of Professional Nursing (Diploma, ADN or BSN program)
-
Current licensure as a Registered Nurse (RN) in the State of Florida
-
Basic Life Support (BLS) Certification through the American Heart Association (AHA) or American Red Cross
-
Valid Driver's License (VDL) and proof of auto insurance coverage
-
OB Homecare: two years of Prenatal Nursing experience in an acute care setting and Current Neonatal Resuscitation Program (NRP) certification through the American Academy of Pediatrics (AAP)
-
Other Homecare: one year of clinical experience as a RN in an acute medical-surgical setting
Preferred qualifications for the ideal future caregiver include:

One year of home health experience
-
Infusion Therapy experience
-
Bilingual (Spanish and Creole)
Physical Requirements:
-
Requires full range of motion, manual and finger dexterity and eye-hand coordination.
-
Requires standing and walking for extensive periods of time.
-
Requires corrected hearing and vision to normal range.
-
Requires some exposure to communicable disease or bodily fluids.
-
Medium Work
-
Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects.
-
Physical Demand requirements are in excess of those for Light Work.
-
Must be ambulatory and able to drive to patients' homes, able to climb stairs, pull, push, and facilitate movement of the patient.

Must be able to read, write and communicate with patients.
Personal Protective Equipment:
- Follows standard precautions using personal protective equipment as required.
