This position is eligible for a sign-on bonus of $10,000 to $15,000 and relocation reimbursement.

As a Clinical Pharmacist in the Transplant department, you will play a vital role in ensuring patients receive their medications accurately, promptly, and with the utmost respect. You will use your expertise to select, dose, administer and dispense medications that support patient health and recovery, primarily focusing on heart transplant and heart failure care. In this role, you will round with heart teams, provide inpatient care, and see clinical patients as needed, while also being cross-trained to support liver and kidney transplant services. A caregiver in this role will work Monday through Friday, 8: 00 a.m. to 4:00 p.m., with rotating weekends every fourth weekend and rotating holidays (one major and one minor). There are no on-call or floating requirements.

A caregiver who excels in this role will:

  • Provide safe, efficient, patient-centered and evidence-based pharmacy and medication services.
  • Ensure the appropriate selection, dosing, administration and dispensing of medication orders.
  • Verify medication orders and/or dispense medications accurately according to state and federal regulations.
  • Document services in a complete, accurate and timely manner.
  • Actively contribute to medication safety initiatives.
  • Ensure all pharmacy operations are in compliance with national accreditation standards.
  • Educate and train students, residents, other caregivers, and/or patients.
  • Provide cost-effective care.

Minimum qualifications for the ideal future caregiver include:

  • PharmD or Bachelor’s of Science in Pharmacy from an accredited College of Pharmacy
  • Current state licensure as a Registered Pharmacist (RPh) or eligible in current state within 90 days of hire
  • Current Basic Life Support (BLS) Certification through the American Heart Association (AHA)
  • Completion of a Board of Pharmacy approved course in the administration of immunizations or certification within 90 days of hire for Individuals administering immunizations
  • Current Advanced Cardiovascular Life Support (ACLS) or Pediatric Advanced Life Support (PALS) Certifications through the American Heart Association (AHA) upon or within one of hire for individuals responding to adult or pediatric cardiopulmonary arrest events

Preferred qualifications for the ideal future caregiver include:

  • Board Certification
  • Completion of a Post-Graduate Year 1 Residency (PGY1), fellowship or relevant post-graduate pharmacy experience in Transplant or Critical Care.
  • Post-Graduate Year 2 Residency (PCY2) in Transplant or Critical Care.

Physical Requirements:

  • Typical physical demands include ability to perform the job while standing and walking for prolonged times, approximately 75% of shift and continuously for extended periods of time.
  • Manual dexterity sufficient to perform computer and dispensing functions of the job; ability to lift up to 30 lbs.
  • Position often includes reaching, stretching, stooping, climbing steps and sitting.
  • Must have normal or corrected vision.
  • Must have ability to clearly communicate verbally by phone or in person
