Outreach Specialist
Location : We are seeking candidates that reside in the Rocky Hill, CT area. This field-based role enables associates to primarily operate in the field, traveling to client sites or designated locations as their role requires, with occasional office attendance for meetings or training. This approach ensures flexibility, responsiveness to client needs, and direct, hands-on engagement.

The Outreach Specialist shall provide individual and family support to ensure members are connected to community services and resources and have the necessary supports. The Outreach Care Specialist is responsible for promoting clear communication amongst a care team and treating clinicians in supporting the individuals and families. Additionally, the Outreach Care Specialist is to coordinate member specific care plans along the continuum. initiatives.
How you will make an impact:

Telephone and/or face to face support to members and families to offer care coordination, support and information. Members may need help navigating the system, completing applications (i.e. Voluntary Services, SSI, System of Care, DDS), locating non –traditional supports, and learning about self-advocacy skills.

Participate in a Local Area Team approach to care management to enhance member treatment by identifying resources, access to care, identifying barriers to care and gaps in service.

Work closely with the Care Management and Peer staff to coordinate services as well as community-based systems of care by leading the facilitation of the Wrap Around Care Coordination model.

Outreach Care Specialists will make home visits; meet with parents; enter documentation in computer system; search databases for appropriate resources.

Collaborate with the Department of Developmental Services, Department of Children and Families, Department of Mental Health and Addiction Services, and the Department of Social Services as necessary to support members.

Build, develop and maintain a positive and collaborative relationship with providers and support organizations by establishing a local presence and by attending meetings.

Outreach to, and work with family and individual advocacy groups across the state.

Work with diverse teams that require independent work initiatives.

Responds to telephone, written, and in-person inquiries and initiates steps to assist regarding issues relating to content or interpretation of benefits, policies and procedures.

Provides timely and accurate resolution of inquiries and issues regarding benefits, services and policies.

Supports and promotes State Sponsored Programs through participation in community events.

Represents State Sponsored Programs in community collaborations.

Supports member access to care through home visits, processing of reports, and distribution of collateral materials.

Performs new member orientations.

Provides superior quality outcomes by taking ownership of issues to ensure timely resolution or follow-up.

Provides superior, professional, and courteous service to customers.

Comprehends the various cultural and linguistic needs of the Medicaid and SCHIP population, knowledge of the various health and social services available in the assigned region with a special emphasis on services offered by community based organizations, ability to work professionally with the company's associates, community-based organizations, providers and plan members.
Minimum Requirements:
- Requires HS Diploma or equivalent and a minimum of 1 year of customer service experience, or any combination of education and experience that would provide an equivalent background.
Preferred Skills, Capabilities & Experiences:

Bachelor Degree in Education, Psychology, Social Work, Nursing, Business, Public Health, Health Care Administration or related field.

Proficiency in the Spanish language desirable.
-
Experience working in the educational or community system with ASD populations or lived experience.

Some combination of the following experiences required.

Experience with Outlook applications and Teams/ZOOM platforms.

Good communicational and organizational skills.
-
Minimum computer skills a plus.

Understanding of the basic principles of managed care and the concepts of publicly financed health insurance such as Medicaid and SCHIP programs is preferred.

Relationships with the community being served and desire to help.
